Our Mission
To provide ambulatory health care, wellness, and related services to anyone, regardless of his or her ability to pay, residing in under served and other designated communities in the Buffalo Niagara region. The Center improves community health through health education and the promotion of health lifestyles.
History
In 1987, Northwest Buffalo Community Center wanted to respond to the health care needs of the Black Rock community in Buffalo. Upon receiving a Grant from the Health Resources Services Administration and the Bureau of Primary of Health Care, Northwest Buffalo Community Health Care Center was formed as a private not for profit.
Northwest Buffalo Community Health Care Center accepts most insurance plans and specifically focuses on the uninsured and underserved. The Health Center utilizes a sliding fee scale structure for our uninsured patients. The scale incorporates the Federal Government's Poverty Level matrix and is updates annually. The patient's household income and the number of members in the home determine co-payment of services. A Co-payment of service could be as little as 10% of the normal visit charge.
