What is the role of health care professionals in the Prescription 4 Health program?

Clinic Health Care Professional

• Posters in the exam room to remind health care professionals of the 4 questions
• Practitioner discusses the 4 questions while completing prescriptions for the medicines
• Pictogram take-away pads available in each examination room are completed by practitioner at the time of writing a prescription for antibiotics and over-the-counter medications.
• The reminder note is attached to each prescription including prescriptions for over-the-counter medications
• Patients are encouraged to visit their community pharmacy and pharmacist and to bring the take-away sheet to the pharmacy.

Pharmacist and Pharmacy

• Posters in the pharmacy to remind health care professionals of the 4 questions
• Pharmacist and pharmacy dispenses the medications and discusses the over-the-counter medications with patient using the same 4 questions with the patient
• Pharmacist provides a copy of the pictogram if needed (pharmacy will have the same pad as in the clinic)
• Pharmacist and pharmacy personnel retains the sticky note for our data collection and places it in the provided marked envelop
